You may be aware that part of the Governments National Programme for Information Technology (NPfIT) includes the eventual uploading of your personal data to a national database.
Many people are concerned about this and the consequences for their private personal medical information. Others feel it could be useful, in particular if you were in the situation of being treated for a serious illness while away from home or out-of-hours.
There is no obligation for you to sign up your personal information to the spine. If you let us know your wishes we can mark your record with a particular code which the Government recognises.
Below is a link to a website that you can download the standard letter opting out of the governments plan.
Complete the letter and send it to us either at the surgery, or by email.
